Liability Agreement

By submitting any form, attending a training session, or contacting us, you acknowledge and agree to the following terms and conditions:

Consent to contact:

I am agreeing that Great Chance Canine may contact you via email or telephone, including text messages at the contact information provided. This consent is not a condition for purchasing services from Great Chance Canine.

Acknowledgment of risks and responsibilities:

  • I understand and acknowledge that there are inherent risks associated with training, socializing, walking and working  with dogs. 

  • I assume any and all responsibility for myself and my dog(s) during any activities involving Great Chance Canine, provided due diligence and care have been exercised.

  • I will not hold Great Chance Canine, or any affiliates or property owners responsible for any injuries, accidents or misfortunes (to or by me or my dog) during these activities, should they occur.  

Health and care acknowledge

  • I understand that my dog(s) may interact in ways that could result in cuts and scratches, and that my participations in these activities carries risks such as tripping, falling, getting knocked down, jumped on, bitten, or scratched.

  • I acknowledge that my enrollment in Great Chance Canine programs relies on my representation that my dog(s) are in good health and have received necessary vaccinations (or equivalent alternatives). 

Emergency situations

  • In case of injury or illness, Great Chance Canine may contact my designated veterinarian. 

  • If the veterinarian is unavailable, Great Chance Canine is authorized to take my dog(s) to the nearest animal hospital for immediate care. 

  • I agree to reimburse Great Chance Canine in full for any medical expenses accrued during their supervision, releasing them for responsibility regarding decisions made by the veterinarian. 

Liability release

  • I release and agree to save and hold harmless Great Chance Canine, any affiliates, or property owners from any and all liability, claims, suits, actions, losses, injuries, or damages caused or sustained by my dog(s) during any activities associated with Great Chance Canine.  

  • I agree to fully indemnify Great Chance Canine for any such liability, claims, suits, actions, losses, injuries or damages.

Financial responsibility

  • I acknowledge sole financial responsibility for any damage or harm caused by my dog(s) during the aforementioned activities. 

Media release and usage consent

  • I consent to the use, posting, and advertisement of any picture, video or recording taken during walking or training sessions by Great Chance Canine.

  • I release any rights to media content, including pictures, videos, audios for Great Chance Canine to use, post on social media or use in any commercial capacity. 

Individual responsibility

  • I recognize the need to ask questions and use my own judgment and comfort level regarding any actions taken during training or related activities.

Payment terms and conditions

  • I agree to all payment terms and conditions outlined in correspondence with Great Chance Canine.

Confirmation

  • I certify that I have read and understood the rules and regulations set forth in this agreement.

  • I  agree to abide by all the terms, conditions, and statements of this agreement. 

  • I confirm the accuracy and truthfulness of the contents of this release form and agree to all payment requirements stipulated by Great Chance Canine in written communications.
